The Montgomery County Mobile Integrated Health program seeks to match high utilizers of emergency medical services (EMS) with more appropriate chronic care and social services offered by public and private providers in Montgomery County. The program takes as its premise that patients who call 911 on a frequent basis have unmet health care or social care needs. The program is a unique partnership between the Montgomery County Fire and Rescue Service (MCFRS) and the Montgomery County Department of Health and Human Services (HHS), as well as several hospitals and other social care agencies. In the 2.5 years of its existence, the program has reduced enrollee EMS transports and corresponding Emergency Department usage by approximately 50%.
