Crow Wing County Community Services’ TIES computer program gives county staff a broader and deeper understanding of clients’ needs, which allows staff to process benefit applications, locate clients and collateral contacts, and respond to requests more efficiently. TIES has allowed for greater collaboration among workers both within and across programs, particularly by facilitating the Community Case Management (CCM) process, during which case managers communicate with one another about the needs of a client in common. TIES has allowed for the identification of 61 clients who have gone through the process to date. During the CCM process, at least seven clients were found to have duplicative services. In addition, the communication and coordination of care has led to better service, reduced the risk of fraud or waste, and improved service levels throughout Community Services.
