In response to the growing needs of its residents, Fulton County has committed to reducing health disparities by addressing the social determinants of health through an interdepartmental collaborative model. The Neighborhood Union Health Center (NUHC) is the pilot site for implementation of the new model of care within Fulton County. The center, a multi-discipline âone-stop-shopâ health center which integrates primary care, behavioral health, public health, and human services at the Neighborhood Union Health Center, is designed with the objectives to increase access to care, reduce unnecessary visits to the emergency room, and to decrease health disparities in Fulton County. Preliminary results indicate NUHC is making positive intermediate steps toward the ultimate goal of improved health. The NUHC Pilot Project serves as a blueprint for the design and implementation of additional comprehensive health and human services centers for other regions of Fulton County.
