To truly change and make health improvements in the local community, the County of San Bernardino, Department of Public Health, must engage in non-traditional partnerships and collaborate and implement public health strategies reaching across all areas of the County. Our Healthy Communities program has successfully worked with several local cities and towns, covering the vast majority of the county population, to improve health. Through an assigned Healthy Communities Coordinator, each participating jurisdiction (city or town) is an active participant in our overall County Healthy Communities program. Leveraging this reach into the community, we conducted a Healthy Communities Evaluation and have created a Health Communities Strategic Plan. These items were complete by utilizing internal and external resources. The future of this endeavor is to incentivize cities and towns in implementing policy aligned with our Strategic Plan and continue to utilize both internal and external resources to do so. In support of the 2018 NACo Achievement Award criteria, the program clearly illustrates collaboration across government jurisdictions, innovation, and has measurable results.
