A recent event at Oakland County Health Division (OCHD) identified gaps in access to HIV care, and the retention in care, for a previously positive client in our community. This event prompted the HIV Department to initiate the HIV Linkage to Care Program in January of 2015. The program helps to identify client’s potential health issues/concerns, strengthens client and provider relationships, and assists with improving the client’s HIV health-related outcomes. The HIV Linkage to Care Program provides an individual needs assessment for clients who are currently living with HIV. The program’s primary goal is to identify gaps in the client’s HIV care, to engage clients into the HIV care continuum, and to partner with local community-based organizations to help facilitate essential support services when indicated.
