Spearheaded by the Mecklenburg EMS Agency (MEDIC), the High Frequency User program is a targeted intervention; designed and implemented as a process to identify high frequency EMS users and manage their care through referral to external resources and the Agency Medical Director. Building on the knowledge that a subset of the population utilizes Emergency Medical Services (EMS) at a higher rate, this intervention established identification, qualification, referral and care plan processes for 19 patients since February 2015.
