The Boulder County Healthy Kids and Medical Home initiatives began in July 2008 to improve child health by linking all eligible children, families, and pregnant women in Boulder County to available benefits and health coverage options. More than 1,600 kids enrolled in BCHK in 2009 and 97 percent of people who apply through BCHK secure coverage from Medicaid or the Child Health Plan Plus. The Medical Home Initiative offers a full spectrum of high-level stakeholders to discuss and act on issues that limit health access to Boulder County’s most vulnerable populations. The program pays a stipend to community-based partners for every application received. Staff works closely with the Boulder County Public Health department through immunization clinics and programs serving children and families, to assure every eligible family is referred and enrolled. The BCHK staff has fostered relationships with a myriad of internal and external agencies to screen and refer clients.
