The Community Transition Center (CTC) is designed to fully support offenders returning to the community from incarceration. The program was implemented in January of 2013 and is co-located with the nonprofit Lighthouse Residential Treatment Program. At this site, a multi-disciplinary team of probation officers, behavioral health clinicians, a nurse case manager and benefits workers assess offenders and link them to services they need to facilitate their reentry into the community, increase their compliance with supervision conditions, and reduce the chances they will commit another crime. The Probation Department transports all offenders to the CTC from every California Department of Corrections and Rehabilitation facility. When offenders arrive at the CTC they are drug tested, and, if positive, immediately referred to on-site detoxification services. If offenders need short-term housing, they can reside at the CTC as long term plans are developed. Offenders who access transitional housing at the CTC receive early intervention services from Lighthouse counselors. Offenders receive transportation from the CTC to a service or program placement or to their homes. The CTC is the only program of its kind in the state, and it helped 1,772 re-enter the community with increased chances of success in its first year of operation.
