The San Bernardino County Department of Behavioral Health’s (DBH) Care Coordination program is a comprehensive care management program designed to assist clients seeking residential substance use disorder (SUD) treatment. The care coordination program is a well-structured service that helps clients access and engage in SUD treatment and other necessary services to overcome barriers and transition between levels of care. Care Coordination program staff meet with clients to identify barriers to a sustained recovery. Once those needs are determined, the care coordinator utilizes a shared decision-making process with the client to develop a plan with goals and steps to prepare for sustained recovery and tools to prevent relapse. The Care Coordinator also assists the client in obtaining needed medical and/or psychiatric clearances, prescribed medications and other services and supports like transportation to intake appointments, so that the client can focus on their treatment. The goal of the care coordination program is to help clients in their recovery journey, to achieve and sustain abstinence, reenter the community and live the life they choose.
