About the Program

Category: Planning (Best in Category)

Year: 2010

More than three years ago, Hamilton County Developmental Disabilities Services (HCDDS) launched a long-term, all-inclusive, customer driven planning process that ultimately became known as What Matters Most, and eventually evolved into Vision 2014. The goal of the process was to publish a five-year strategic plan that focused better than had ever before been imagined on what was most important to individuals with disabilities and their families. The plan was to be the agency’s guiding document for service delivery for six years and the framework for all decision making action. Work groups focused on streamlining processes and timelines for team decisions about funding, offering support for home options of in communities of the individuals choosing, researching home availability that meet an individual’s dreams in the community of their choosing, matching an individual’s interests and gifts to community organizations or volunteer opportunities to decrease isolation and increase community involvement and developing methods for individuals with disabilities who are not able to communicate through speech to voice their opinions and beliefs in meaningful ways. Overall, this project gathered specific information in comprehensive and effective ways so that the strategic plans and annual action plans are more accurate and direct reflections of what truly matters most to individuals with disabilities and their families.
