San Bernardino County’s Department of Adult and Aging Services-Public Guardian (DAAS-PG) developed the Adult Protective Services (APS) At-Home program under the umbrella of the Home Safe program established by California Assembly Bill 1811. At-Home aims to assist APS clients experiencing homelessness or those at risk of homelessness due to elder or dependent adult abuse, self-neglect or financial exploitation through the provision of case management services and short-term financial assistance.Successful techniques have involved collaboration with other County departments and community partners and the creation of program materials for staff and clients to support outreach and service delivery. From July 1, 2019 to December 4, 2020, the At-Home program has assisted 9,530 individuals through housing stabilization support in the form of moving costs, rent and utility grants, room and board, gift cards, rental security deposits, food supply, resource referrals, transportation, housing navigation, case management and home improvements and repairs. The San Bernardino County DAAS-PG At-Home program has prevented homelessness for thousands of vulnerable elders and dependent adults, and minimized the risk of homelessness for many others.
