The Healthy Living Collaboration is a public private partnership between the Maricopa County Department of Public Health (MCDPH) and Dignity Health. The partnership was formed in March of 2016 to plan and implement the Stanford Small-Group Self-Management Programs in Maricopa County. The Stanford Small-Group Self-Management Programs are a tested and evaluated self-management program for people with chronic health problems. The programs are designed to help people with chronic disease and their caregivers gain confidence in their ability to manage their condition through symptom control and understanding how their condition impacts their lives. This program has been shown to be safe and effective. Participants demonstrate significant improvements in the areas covered and report fewer hospitalizations and outpatient visits. The collaboration provides both entities with resources necessary to hold workshops. Each workshop consists of a minimum of six classes held weekly. Each class lasts approximately 2 hours and is designed to host 12-20 individuals. Workshops require two Lay Leaders who have received comprehensive training and ongoing evaluation. Additionally books and other supplies are necessary to hold a successful workshop. Each workshop requires planning and coordination to be successful. A location and date for the classes are necessary as well as facilitators and books. The workshop needs to be properly promoted within the community to ensure attendance. The outcomes for the program include the number of classes held and the number of participants that complete each workshop. To date, eight workshops have been completed, three are in progress, and five more are planned before the end of the fiscal year on June 30, 2017. Over 70 individuals have completed the workshops since July 1, 2016.
