This data use agreement (DUA) is between the King County Mental Health, Chemical Abuse and Dependency Services Division and a data receipent for the use of a limited data set (LDS). The LDS excludes direct identifiers such as name, address and Social Security number, but the DUA allows a data receipent to use some identifiable information such as zip code, admission, discharge and service dates, birth date and age.
