Building a Culture of Health through Cross-Sector Collaboration
Transportation, social support networks, employment, education, housing and income contribute to up to 40 percent of a person’s overall health, and county policies can significantly impact these social determinants. Many counties have fostered collaboration across the numerous sectors that affect social determinants in order to build healthier communities. These counties will share how they expanded the definition of health in their counties to create a county-wide culture of health.

Transportation, social support networks, employment, education, housing and income contribute to up to 40 percent of a person’s overall health, and county policies can significantly impact these social determinants. Many counties have fostered collaboration across the numerous sectors that affect social determinants in order to build healthier communities. These counties will share how they expanded the definition of health in their counties to create a county-wide culture of health.
