The Camino de Salud Network is a public-private partnership formed in 2004 to address the challenges of fragmentation and coordination of care for safety net patients. The Coordinating Care for the Uninsured is managed by COPE Health Solutions, a non-profit health care corporation with a mission of “Making Communities Healthier”, and currently includes the LAC+USC Healthcare Network, Barlow Respiratory Hospital, Citrus Valley Health Partners, and twelve community clinic organizations. The focus of the CDSN is to enable access to the “Right Care at the Right Place at the Right Time.”
