Hennepin County’s Human Services Case Management Redesign Project is improving the cost effectiveness of the Department’s case management services by reducing redundant assessment activities and triaging cases to the appropriate level of service. Electronic case files will allow staff to more efficiently access assessment and case plan information. Tools to track case manager productivity and expenses allow them to consistently measure the time case managers spend on client contact versus administrative tasks across all service areas, maximize federal dollars for social services case management and use fewer property tax resources to support core work. Going beyond complying with the federal DRA requirement of one case manager per client, they are implementing a functional approach to managing their work. This requires assigning managers to be responsible for functions across all service areas, rather than being responsible for only a single service area or program. The Human Services Case Management Redesign Project enhances their ability to work in integrated teams and management groups, requires them to seek input from all who are impacted by their work inside and outside the department, and provide transparent easily accessible case management services to the community.
