Initiated in 2009, Multnomah County’s Homeless Benefits Recovery (HBR) Program has achieved outstanding results in securing Social Security Income (SSI), Social Security Disability Income (SSDI), and Medicaid as well associated federal benefits for disabled individuals who are homeless or at risk of homelessness. HBR works with individuals to successfully navigate the complex process of applying for SSI/SSDI and Medicaid benefits, including: Obtaining the medical records and necessary medical and psychological information, completing application paperwork, arranging transportation to related appointments, and representing applicants at Disability Determination Hearings. Highly trained Benefit Specialists have been the engine for the program’s success. At the end of FY 12, 82% of clients who exited the Homeless Benefits Recovery Program secured federal SSI or SSDI disability benefits. This number far exceeds other most benefit recovery models (10% - 15% success rate) around the nation.
