Hennepin County’s integrated health care delivery network is designed to serve the unique needs of one of the most challenging and costly segments of the county’s safety net population. By integrating medical, behavioral health, and human services in a patient-centered model of care, the project seeks to improve health outcomes dramatically and lower the total cost of providing care and services to the Medicaid Expansion population. The project went live January 1, 2012 and has over 6,000 enrollees. Hennepin Health has implemented a number of initiatives which have improved systems, reduced overall costs and improved health outcomes. In the first year of operations, many system changes have been implemented with costs reduced by >5% below projections. These improvements have also changed individual care by reducing crisis episodes and increasing preventive services. Outcomes (Jan-Aug 2012): – Admissions: Decreased 29% from 17/1000 members to 12/1000 members – Readmissions: Decreased 2% - 5% (hospital specific variance) – Emergency Dept.: Decreased 35% (includes urgent care change) – Primary Care: Increased 23% (for consistent enrollees) – Patient Satisfaction: 87% “likely to recommend” (Press - Ganey) The impact in year one has been significant and shows continued promise in year two for further cost reductions and system reinvestments.
