The County of San Diego built a leading-edge computer system called ConnectWellSD to improve customer outcomes through innovative data sharing. ConnectWellSD integrates data from eight County departments and two community service provider directories to create a comprehensive record for each customer, which allows service providers a holistic view of their customer’s history and needs. Additionally, ConnectWellSD serves as a secure data platform on which service providers from across disciplines can collaborate on a single customer case to more efficiently deliver integrated and comprehensive services to customers. This enhanced customer service is called person-centered service (PCS). PCS is a holistic approach to service delivery that takes into account the various needs of customers, connects individuals to the resources they need, and promotes collaboration across disciplines as needed for positive customer outcomes. ConnectWellSD links service providers working in housing, mental health, benefits, public health, alcohol/drug services, child welfare, probation, and aging services. The initial release of ConnectWellSD spans from June 2017 through May 2018, and will result in more than 5,000 connected users.
