Ryan White HIV/AIDS Program (RWHAP) program data indicates that people living with HIV who are temporarily or unstably housed have lower rates of retention in care and viral suppression. Increasing access to and participation in health care through housing benefits patient health outcomes. Integrating housing and care data sources helps to facilitate patient linkage, navigation, and service coordination across community and health systems, subsequently improving health outcome along the HIV care continuum. Through funding for this project is provided by the Health Resources and Services Administration (HRSA) HIV/AIDS Bureauâs (HAB) Special Projects of National Significance (SPNS) Program, in collaboration with the Department of Housing and Urban Developmentâs (HUD) Office of HIV/AIDS Housing (OHH) led by RAND corporation; Palm Beach County, Community Services Department, Ryan White Part A Program; and the City of West Palm Beach, Department of Housing and Community Development, HOPWA Program. Project partners have developed, implemented, and evaluated an integrated data system to share cross program client information and coordinate HIV care and housing services across multiple funding programs to enhance and streamline local HIV service coordination along the HIV care continuum.
