    San Bernardino County, Calif.

    Connections is Mariposa Community Counseling’s services hub for our clients. It is a centralized approach to connecting clients to services throughout the West End. Clients come to CONNECTIONS to learn about the therapy and case management services that are available to them in the clinic and in the community. It is unique in that we partner with South Coast Community Services, our DBH contract clinic, to also be in attendance to offer services that are available in Upland and Chino. This prevents clients from bouncing around and increases access to our clients. Our clients are able to schedule directly with them, decreasing the time it takes to enter into treatment. This centralized approach and partnership is unique in its collaboration and approach to client care.

Through this initiative, NACo worked with county leaders and partners to share best practices that bolster our nation’s ability to thrive county by county.
