Register to attend NACo’s 2019 Healthy Counties Forum on April 23. Opt into even more learning opportunities at San Diego County’s 2nd Annual Community Information Exchange (CIE) Summit when you complete your forum registration.
NACo’s Healthy Counties Initiative works to create and sustain healthy counties, supporting collaboration and sharing innovative approaches to pressing health issues. In recent years, participants have explored social determinants of health, the opioid crisis and the intersection between community health and justice sectors.
This year’s Healthy Counties Forum will provide tools and strategies in support of counties’ efforts to uniquely define health equity by looking at a baseline of local assets and barriers. Registrants will engage with policy experts, health providers and thought leaders, and will participate in a hands-on workshop to learn how to use available data and free online tools to identify health disparities, build real-time snapshots and use the information to forecast change, increase coordination and evaluate of local strategies.
This full day of programming will begin in the morning on April 23, so participants are encouraged to arrive the evening before. We ask that participants bring their laptops with them to the Forum, as a laptop is required for one of the workshops. A detailed agenda will be posted online soon.
San Diego County is offering complimentary registration to its Community Information Exchange (CIE) Summit on April 24 to all county representatives participating in the Forum. NACo encourages you to stay another day to learn:
- How to create an upstream savings and return on investment through creating sustainable efforts in addressing social determinants of health
- How to engage in systems change that creates movement towards person-centered care and cross-sector collaboration
- Learn about informed care: legal, privacy and human-centered ethical standards through best practices, policies procedures and approaches
- Learn how to build infrastructures for data sharing with technology platforms, information sharing and informatics that drive the evolution of health and social services
