Description

Re-entry services: Mental health and substance use disorder case managers coordinate with jail staff to screen individuals for treatment needs, stable housing, income assistance, medical care, and other services, prior to release from detention. Coordinating resources and developing an effective, personalized re-entry plan with individuals before their release increases their stability and safety in the community and reduces re-arrest rates. Diversion services: Persons with severe mental illness are identified and evaluated, and a community linkage plan is submitted to the judge at the first appearance or bond hearing. If approved by the judge, the defendant is released with case management supervision and engaged in community treatment.
